PLEASE READ THIS DOCUMENT CAREFULLY. BY ACCEPTING AND SIGNING, YOU WAIVE CERTAIN LEGAL RIGHTS.
1. Participation: I hereby authorize the registered athlete to participate in all trials, tryouts, physical exercises, and activities organized by PrimeX League. I acknowledge that sports activities involve risks of severe injury.
2. Assumption of Risk: I voluntarily assume all risks associated with participation, including but not limited to physical strain, collisions, field conditions, and weather exposure.
3. Waiver & Release: I release, waive, discharge, and covenant not to sue PrimeX League, its staff, coaches, volunteers, and host facilities from any liability, claims, or injuries arising from participation.
4. Medical Release: I authorize PrimeX staff to seek emergency medical treatment for the athlete in the event of injury or illness when parents/guardians cannot be contacted immediately.
